2024-25 FORMS and DOCUMENTS TO REVIEW

Athletic Participation/Parental Consent/Physical Examination Form – required for any student who plans to participate in any TNCS sport during the school year. Physicals must be dated after May 1, 2024.

Fall Athletics and After School Activities

MEDICAL FORMS

Immunization Form – required for all rising 7th and 12th graders and all new students.

Medication Authorization – required for all students who plan to have prescription medication administered during the school day.

Asthma Action Plan – required for any student diagnosed with Asthma.

Allergy Action Plan – required for any student diagnosed with Allergies.

Diabetes Action Plan – required for any student diagnosed with Diabetes.

Seizure Action Plan – required for any student diagnosed with Seizures.
